Indiana Wholesale Customers

We have been busy at Nutritional Resources updating our customer records to ensure that our wholesale customers are correctly filing for sales tax exemptions.

We wanted provide an opportunity for your business to submit the proper form to claim a tax exemption. If we do not have a valid tax exemption from your business, your orders with Nutritional Resources will be charged current tax rates for your state. 

What we need from you: 

  1. Wholesale customers in Indiana need to submit form ST-105. Click this link to download the form: Indiana General Sales Tax Exemption Certificate (ST-105).
  2. Please complete the form with your information.
  3. Email a copy to wholesale@nutritionalresources.com.

If you have already submitted this form to claim your sales tax exemption, you can disregard this email.
